In processing this request, you certify you are an authorized representative of the entity which maintains ownership of the content being modified. Once the form is submitted, you'll receive an agreement via e-mail. You must respond and agree to the statement in order to proceed.

Claims Released

In order to proceed, you must release any existing claims on the video within YouTube CMS before submitting this request. If this is not completed, the request will be cancelled.

This does NOT apply to in-network CHOWNs (ArtistVEVO to ArtistVEVO CHOWNs)
